Social Work (60 minutes) Review session to follow up after assessment.
Service Description
One-on-one session of supportive social work with your qualified social work practitioner. Following the confidential psychosocial assessment which outlined problem areas of your life, spend time with your social worker evaluating the support plan which has been made available to you. Make purposeful choices in a non-judgemental and supportive space, reviewing your support plan or putting in place a maintenance plan towards ensuring balance in the next chapter of your life. You can book this 60-minute session for an in-person, online or telephone session.
Terms and Conditions
* Following your booking, you will receive an appointment confirmation via email and SMS. * If this is your first session, the email will also provide you with a link to complete our Intake Form. * We understand that, from time to time, things can come up that require you to reschedule your appointment. If you are unable to attend an appointment in person, your session can be changed to an online appointment. Please call or email if you are unable to attend your appointment at all and you need to cancel the booking. You can leave voicemails any day of the week if it is outside of our usual business hours. We do not accept SMS cancelation or rescheduling. Cancellation fees apply for no-shows. * Please refer to our online "Payment Terms and Conditions" for information on payments, bookings and cancellations.
