Care and Support for Older Persons

Growing older is a privilege, and everybody deserves to feel supported, respected and understood as life changes around them. Maybe some things take more energy than they used to — perhaps because of health concerns, reduced confidence, emotional strain or general frailty. Perhaps everyday tasks are becoming harder, and even when family want to help, they may be juggling their own responsibilities and unable to offer the level of support you need.

You might also find yourself holding back from asking for help. Many older people tell me they want to stay independent, avoid burdening others, or worry that accepting support could lead to residential care. These feelings are very common in our Elder communities, and it’s completely understandable to feel this way.

At Thoughtful Connections, our role is to walk alongside you through these changes. We offer practical assistance, emotional support and counselling to help you adjust to shifting health, loss, grief or bereavement. If you’re hoping to stay at home, we can help you connect with My Aged Care and explore services that make daily life safer and more manageable. And if remaining at home is no longer the safest option, we can support you through the transition into residential care in a way that feels respectful, steady and considered.

Therapeutic Counselling

To care for your emotional and mental health, we offer therapeutic counselling to support you through life transitions, change, loss, grief and bereavement. You’ll have someone in your corner when you’re feeling powerless or overwhelmed, with advocacy and steady emotional support to help you regain a sense of control. We can also assist with mental and emotional health concerns and connect you with other professionals if additional care is needed. When relationships feel strained — whether with family members, intimate partners or other important people in your life — we provide relationship counselling to help strengthen communication, rebuild connection and support healthier ways of relating.​​​

​​​​We can help you make sense of these and other services or support systems around you, to ensure you’re not navigating them alone. With your consent, we can act on your behalf to help:

  • arrange My Aged Care assessments and referrals to home‑service providers

  • navigate systems such as My Aged Care, Department of Housing, Centrelink / Services Australia, and QCAT for guardianship or administration orders

  • understand Advance Care Planning, including Wills, Enduring Power of Attorney (EPOA), Advance Health Directives (AHD), and Statements of Choices (SOC)

  • manage hospital admissions or discharges, retirement planning, respite care, or transitions into permanent residential care

  • organise meetings or mediation with family, neighbours or formal agencies.​

Working with your nominee

You may prefer a trusted family member or other decision-maker, such as an Enduring Power of Attorney, to work with us to support you. As long as we have your consent to do so, this does not present a problem.  

Funding

Social Work services can be funded privately or through programs such as Medicare, My Aged Care, DVA, or some private health funds. Do not let financial pressure be the reason you don't reach out to us - we offer fee reductions to concession card holders and we are happy to discuss your special circumstances.

Our Social Worker follows a strict ethical code. Please reach out if you are not coping at home, not receiving proper care, being harmed or abused, or are becoming socially isolated. This also applies if you know another older person who is struggling like this. We are happy to work with you or them — and with anyone involved in their care — to complete a comprehensive psychosocial assessment, identify support needs, and put safe, appropriate community supports in place. 

Nobody needs to struggle alone.
